Here’s generally how phalloplasty works:

Skin grafts are usually a synthetic skin graft such as Alloderm or some other sort of synthetic skin-like material which is often also used in reconstructive surgery.  It’s safe, so that’s good. 

Usually a device is inserted into the penis to get it to achieve the length desired.  I believe this is usually made of some type of body-safe plastic as well, and is inserted through an incision into the penis.  I know, ouch.

2.  Penis widening.  This ones a little more interesting, and perhaps the more complicated of the two.  Instead of lengthening the penis, this is where the actual girth of the penis is increased through artificial means. 

Penis widening is done through fat grafts that are inserted into the penis to achieve the “fatness” of the penis desired. Many men can enjoy up to a fifty percent increase in their penile girth when the penis is erect, and anywhere from five percent to thirty percent noticeably visible increase in the girth when the penis is in a flacid state.
